Remember, there are four forms that need
to be completed and sent: The Request
for Reservation Sponsor’s Form, Information Form and Release of Liability (Disclosure Form).
Also, Chrysalis applicants must be 15 years old or
have completed the ninth grade. We can accept those who fall into the Journey age group. If you have any questions, please
